If you need to cancel or reschedule, please call us at (937) 504-9999 as soon as you can. Booking an appointment does not obligate you to enroll in any plan.
Text Message Reminders
If you agree to receive texts, we may send appointment reminders and messages about your scheduled appointment, sent by an automated system. Message frequency varies. Message and data rates may apply. Reply STOP to opt out at any time, or HELP for help. Agreeing to receive texts is not a condition of any purchase. Carriers are not liable for delayed or undelivered messages.
