Follow the instructions to get your

Current Medicare number & Card if you don't have it!

1. CALL 1-800 MEDICARE
2. When the automated announcement ask, When the automated announcement asked at the beginning,

        Tell me what are you calling about?  Say, " I NEED MY MEDICARE NUMBER"

        (You will be transerred to a live person)

    3. Tell the Representative, " I NEED MY MEDICARE NUMBER ALONG WITH MY PART A & PART B EFFECTIVE DATE.                  I ALSO NEED A NEW CARD."

        (they will verify who you are and then give it to you over the phone. WRITE IT DOWN

   4.  Call our office right away!

      We need your current Medicare number on file and need        it to process your Health Questions with Anthem.
